Please review the guidelines in this document to see if your event qualifies as a Community Partner Fundraiser. If you are interested in entering into an agreement with Komen San Francisco, please fill out the Third party event request form and submit it to admin@komensf.org. You may also fax the completed form to 415-397-8817 mail to the SF Bay Area Affiliate of Susan G. Komen for the Cure, 26 O'Farrell Street, Suite 900, San Francisco, CA 94108.
